ATTENTION COACHES AND VOLUNTEERS!

PoncaSoccer is REQUIRED to have a background check completed (annually) for ALL Volunteers (over age 18) who will be interacting with children.

Oklahoma Soccer Association is using GotSport to conduct the background checks. ALL Volunteers (over 18) need to be entered into GotSport. Once entered, the volunteer is assigned a username and password so they can log in and complete the Background Check questionnaire (an email address is required). Alternately, volunteers and coaches may fill out a Volunteer Disclosure Form and give PoncaSoccer permission to enter their information for Background Check.

This is a US Youth Soccer requirement intended for the protection of all our children.

PoncaSoccer still does not have completed forms from many of the Recreational Coaches and volunteers. These MUST be filled out and returned to PoncaSoccer ASAP. Forms will be available at the soccer complex concession stand, and can be returned to the Referee Shack.

If you prefer, you can print out your VDF from oksoccer.com. If you go to www.oksoccer.com and look on the right side, you will see a link that takes you OSA VOLUNTEER DISCLOSURE FORM. Click the link and follow the instructions.

Thanks for taking the time to fill out the REQUIRED form.
